PRIMARY DUTIES AND RESPONSIBILITIES:

The New York State Office for People With Developmental Disabilities (OPWDD), has developed a new program model called Comprehensive Adult Transitional Homes (CATH). These are State Operated Individual Residential Alternatives (IRAs), that are designed primarily to serve individuals who are transitioning from residential schools into residential programs within NYS OPWDD. These programs create a service pathway accessible to those eligible for OPWDD services and determined in need of these programs by the Statewide Admission Committee (SAC). This pathway begins with a clearly established referral, screening, and intake process. Once admitted they will participate in approximately 18 months of continued treatment, stabilization, and integration towards there longer-term living plan.

OPWDD CATH provides specialized psychiatric and behavioral habilitative services for up to 6 individuals at any given time. Services provided in CATH are person-centered and maintain a focus on continuous assessment, active treatment, and discharge planning to assure a successful return to the individual’s home or community-based setting following treatment. These individuals will be coming from residential school settings with a significant amount of structure and will require increased staffing resources to offer individualized attention.

Candidate will work as a Residential Registered Nurse I. The candidate will provide oversight to individuals with developmental disabilities who have a variety of other medical and psychological diagnoses. This person will have a key role in supporting the individuals at the first CATH home in Capital. As this program is developing, this person will also have responsibilities assisting other Residential RNs within the 3 covering counties with nursing duties as assigned.

  • Candidate will function as a part of the Intensive Support Team.
  • Ensures that medications and treatments are administered according to approved policies and regulations. Provides training to staff administering medications, retraining in medication administration practices as needed and delegated nursing tasks.
  • Performs medication audits and resolves any issues found, conduct OSHA audits, oversees weekly med room checks and oversight of medical supply ordering in nursing supported IRAs all as needed.
  • Assist supervision of overall individual's health care needs and works with house management to implement optimal care. Assess individuals on an ongoing basis, identifying needs and provides medical follow up (work with house leadership and Residential nurse to coordinate appointments for primary, consultant services and required tests/diagnostics if needed).
  • Assist to ensure Nursing plans of service, physicians’ orders, necessary consents (MIPS/Informed Consent/upcoming procedures) and related documentation are up to date.
  • Completes, at minimum, one visit weekly to the Nursing supported IRAs and CATH IRAs to complete oversight duties.
  • May assist Hospital Liaison with hospital coverage with providing oversight during shift to individuals when hospitalized through daily report updates and will facilitate smooth discharge transitions by collaborating with hospital, treatment team and local distributors to fulfill medical needs upon discharge.
  • Ensures that medications and treatments are administered according to approved policies and regulations. Provides ongoing oversight and training to staff administering medications, retraining in medication administration practices as needed and delegated nursing tasks.
  • Provides nursing component to the Interdisciplinary Team. Collaborates with clinicians, team members, families, and the medical community to provide optimal care for the individuals.
  • Provides additional coverage for other caseloads as needed.
  • May cover LPN shifts as needed within RN’s coverage area to ensure continuity of medical care for individuals.
  • May provide supervision of LPN positions if determined necessary.
  • Incident Investigation and other duties as assigned.

The Candidate must be able to perform CPR compressions on a supine individual on the floor. The Nurse demonstration of CPR compressions on the floor would ensure nurse ability to perform in worst case scenario, since we use no backboards in OPWDD.

Minimum Qualifications

MINIMUM QUALIFICATIONS:

  • Permanent service as a Registered Nurse 1

OR

  • Eligible to 70.1 transfer into a Registered Nurse 1 title.

OR

  • Open to the Public: Licensed and currently registered to practice nursing in New York State.
  • Must maintain a valid New York State license and registration

ADDITIONAL COMMENTS:

  • Please note that starting salary for this title reflects the increased hiring salary and geographic pay differential. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 18 ($66,951), Increased Hiring Salary ($9,196), and Geographic Differential ($20,000). Geographic Differential added to the job rate ($85,138) for PS&T Grade 18.
  • This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.
  • At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
  • Family and household member relationships: OPWDD policy may impact eligibility for this position.
