POSITION DESCRIPTION:

Registered Nurses 2 (Community) are registered professional nurses who provide, oversee, coordinate, and evaluate nursing and health related care and treatment services for individuals* in community settings. Incumbents serve as members of interdisciplinary treatment teams; represent and provide expertise on the nursing aspects of treatment plans and services; and provide nursing services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders and their caretakers or families.

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Functions in the role of an RN 2 (community) as assigned, including oversight of family care homes, day habilitation sites, and IRA settings.
  • Provides health education and training for individuals, families, and providers/staff; and performs appropriate interventions in the event of a crisis.
  • Provides training on various nursing topics such as medical emergencies, non-emergency medical situations, medication administration, and documentation to family care providers and unlicensed/licensed direct care staff, to ensure that quality nursing services are provided to individuals.
  • Assists Nursing Administration in identifying and addressing knowledge deficits and/or documentation vulnerabilities, as it relates to nursing, across the DDSOO; which will include preparing and submitting data and reports for administrative review, routinely.
  • Provides training and mentoring to Nursing department staff as necessary, such as how to write an effective Plan of Nursing Service, MOLST, SDMC process, utilization of the electronic health record/e-MAR, case management, etc. This will include on-site shadowing and remote support to nurses.
  • Assists in communicating with medical providers, community vendors or agencies, when necessary, to promote quality services.
  • May supervise and counsel lower-level staff and participate in routine chart audits, site reviews, and feedback sessions.
  • Participates in various agency committees and assists in the ongoing revision/review of training materials as needed.
  • Other duties as assigned.

Ability to adjust schedule to meet operational needs is essential, as is willingness to travel to various worksites as assigned.

Minimum Qualifications

Applicants must possess a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.

Substitution:

A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.

Please note that starting salary for this title reflects the increased hiring salary and geographic pay differential. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 20 ($74,193), Increased Hiring Salary ($9,980), and Geographic Differential ($20,000). Geographic Differential added to the job rate ($94,121) for PS&T Grade 20.

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

Posted Hiring Salary includes the sum of the statutory hiring rate ($72,032) for PS&T Grade 20, Geographic Pay Differential ($20,000) and Increased Hiring Salary ($9,688). Maximum salary includes the sum of Job Rate ($91,381) and Geographic Pay Differential ($20,000).

  • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
  • The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
  • Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume.
  • All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.

The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer. ° Thirteen (13) paid holidays each year.

° Thirteen (13) days of paid sick leave each year (PEF/CSEA), which may carry over from year to year.

° Up to three (3) days professional leave each year to participate in professional development events.

  • Paid Parental Leave:

Paid Parental Leave allows eligible employees to take up to 12 weeks of paid leave at full pay for each qualifying event. A qualifying event is defined as the birth of a child or placement of a child for adoption or foster care.
