Job Title: RN – Case Manager

Start Date: 06/30/2025

End Date: 09/01/2025

Shift: 5x8 Days (8 hours per shift)

Pay: $3,000/Week

Location: Bronx, NY

Position Overview:

We are seeking a dedicated and experienced Registered Nurse (RN) – Case Manager to join our healthcare team in the Bronx, NY area for a short-term contract. The ideal candidate will play a vital role in coordinating patient care, managing transitions, and ensuring optimal patient outcomes throughout the continuum of care.

Responsibilities:

  • Perform assessments of patients to identify needs and coordinate appropriate care plans
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals
  • Monitor and evaluate patient progress, revising care plans as needed
  • Educate patients and families about care plans and available resources
  • Facilitate discharge planning and follow-up care coordination
  • Ensure documentation is accurate, timely, and compliant with regulations and standards
  • Advocate for patients’ needs while balancing clinical and cost-effective outcomes

Requirements:

  • Active RN license in New York
  • Minimum of 2 years of case management experience
  • Strong knowledge of care coordination, discharge planning, and utilization review
  • Excellent communication and organizational skills
  • Experience with electronic medical records (EMR) systems
