Position Description

Title: Registered Nurse - Primary Care

Location: Northeast Region (Onsite)

Employment Type: Contract, Mon-Fri, 40 hours a week

Compensation

Pay Range: $50.00-$52.00 Per HR

Description

Provide primary care to patients, including assessment and treatment.

Develop care plans and evaluate medical care.

Maintain licensure and participate in ongoing education activities.

Support patients with self-care management and follow up on lab results.

Handle medication refill requests and coordinate treatment plans.

Provide direct care, such as wound care and medication administration.

Monitor patients awaiting transport to emergency services.

Key Responsibilities

Review work and develop plans based on patient needs and team skills.

Support implementation of organizational programs and procedures.

Collect data and adjust processes to meet program requirements.

Participate in program evaluation activities.

Practice in accordance with nursing standards and safety procedures.

Provide direct patient care as necessary.

Develop strategies to maintain wellness for at-risk populations.

Qualifications

Completion of nursing training from an accredited school of nursing.

Minimum of one to two years' experience in a medical practice or primary care setting.

Current Registered Nurse license in New York State.
