Position Description

Job Title: RN Case Management - Local Contract

Duration: 08 Weeks

Start Date: 06/30/2025

Location: Bronx, NY

Gross Weekly: $2,560/40 Hours

Shift: Monday-Friday LOCAL CANDIDATES ONLY

Requirements:

  • Two years case/resource management and utilization management is required
  • Required: New York State RN License, Primary Source Verification, AHA BLS
  • Must have Hospital Inpatient Care Management experience

Duties:

  • Assess patients upon admission to identify clinical, psychosocial, and discharge planning needs
  • Coordinate care with interdisciplinary team (physicians, nurses, social workers, therapists, etc.)
  • Initiate and manage discharge planning early in the patient’s hospital stay
  • Arrange post-acute services (home health, rehab, skilled nursing, etc.)
  • Ensure appropriate level of care and transitions between care settings
  • Conduct utilization reviews to ensure medical necessity and appropriate resource use
  • Communicate with insurance providers for authorizations and prevent claim denials
  • Document all case management activities accurately and in a timely manner
  • Ensure compliance with hospital policies, regulatory standards, and accreditation requirements
  • Advocate for patients and families, providing education and support throughout hospitalization
  • Participate in interdisciplinary rounds and collaborate on care goals
  • Track patient outcomes and participate in quality improvement initiatives
