Position Description
The Pathways Program supports clients transitioning from psychiatric institutions into supportive housing and the community. We are seeking a Registered Nurse to conduct assessments, provide basic health education, coordinate care, and support clients across the Bronx.
Parking is available onsite, with the option to receive a MetroCard or mileage reimbursement for community visits.
Location: Bronx, NY (Onsite, 3 days/week + 2 remote)

Pay Range: $50.00-$55.00 Per Hr

Key Responsibilities
- Conduct mental & physical health assessments within 30 days of enrollment.
- Provide basic medical education (diet, glucometer use, cane/walker instruction).
- Attend first medical appointments and assist with scheduling ongoing care.
- Perform community visits 2-3x/week (mostly within the Bronx).
- Participate in new client sign-ups at hospitals such as Bellevue & Metropolitan.
- Complete virtual assessments when appropriate.
- Coordinate services with case managers, clinicians, and peer specialists.
- No medication administration or LAI injections.
- Complete documentation
- Client Population
- Individuals being discharged from psychiatric institutions.
- Clients living in Bronx supportive housing or residences in Bronx
- Low-volume caseload (2-3 clients per week)
- Active NYS RN license.
- Experience with assessments, case management, or behavioral health preferred.
- Strong communication and organizational skills.
- Spanish bilingual a plus.
