Med-Scribe, Inc. is seeking experienced RNs to join a dedicated group that shares your passion for helping the community.  This is an exciting long term temporary position with a very strong potential for hire located in Fairport, with future work from home options.  If you are seeking to use your clinical experience in a more administrative capacity, this is the opportunity for you!

As a Medical Case Manager, you will strive to improve clinical outcomes and provide cost-effective services for patients.  By using a combination of research, investigation, and patient health education, you will conduct care management program activities and provide care coordination and treatment options. In doing so, you will provide linkage to appropriate community resources; make referrals to appropriate care services, and communicate with providers in order to improve care outcomes. The majority of the work is telephonic, however you may be asked to travel to client’s homes to assess their care, however there is no direct patient care.

Schedule: Full Time, Monday to Friday, 8:00AM-5:00PM.
Salary: $30/hour
Benefits: Medical and Vision Insurance, paid holidays and PTO.

Minimum Qualifications:
”    A valid NYS RN license is required.
”    A minimum of three years clinical experience is required.
”    Excellent written and verbal communication skills are required.
”    Previous case management or community health experience is an asset.
